1) Effective May 5, 2020, we no longer accept missing receipts in lieu of the actual receipts for the purchases / services from online vendors such as Amazon, Dropbox, etc. Please log into your online vendor account to obtain the receipt you are missing
2) For all other purchases / services for which the receipt is missing, reach out to the vendor to obtain the original receipt. If the vendor is unable to re-issue the receipt, complete the missing receipt form and provide the detailed explanation regarding the items/services purchased and the explanation on why the original receipt is not available
